For New York City Residents

If you live in the New York City (Manhattan, Brooklyn, Queens, Bronx and Staten Island), you should receive your renewal documents in the mail directly from the New York City Human Resources Administration (NYC HRA). It will arrive with a self addressed stamped envelope. Our plan can help you to renew your Medicaid Coverage.  When using any of the methods below you will want to have:

  • Proof of all types of income (social security, retirement, disability, workers compensation, etc.).
  • Current month banks statement for all accounts (ONLY IF APPLICABLE)
  • Proof of cost for Supplemental Medical Insurance (ONLY IF APPLICABLE)
  • Verification of Deposit from the Pooled Income Trust Company (ONLY IF APPLICABLE)

 

For Nassau County Residents

You should receive your renewal documents in the mail directly from the Nassau County Department of Social Services. Our plan can help you to renew your Medicaid Coverage using several options. When using any of the methods below you will want to have:

  • Proof of all types of income (social security, retirement, disability, workers compensation, etc.).
  • Current month banks statement for all accounts (ONLY IF APPLICABLE)
  • Proof of cost for Supplemental Medical Insurance (ONLY IF APPLICABLE)
  • Verification of Deposit from the Pooled Income Trust Company (ONLY IF APPLICABLE)

 

For Westchester County Residents

You should receive your renewal documents in the mail directly from the Westchester County Department of Social Services. Our plan can help you to renew your Medicaid Coverage using several options. When using any of the methods below you will want to have:
